Requirements For SHS Enrollment

Requirement No. 1

Duly - accomplished completed SHS Enrollment Form

Requirement No. 2

Birth Certificate
(If not available during the enrollment you may submit it within the school year.)

Requirement No. 3

Report Card, Form 138 or any document proof of completion of Grade 10 in any public or private secondary school.

Requirement No. 5

Medical Certificate

Note:

Students are also advised to bring their NCAE results during enrollment so they can be guided accordingly on the choice of school, track and strands.
